From my perspective, the Caldicott principles were written for a paper-record world, but they translate cleanly to the Ai era. Specially when Ai is used for consultation notes, referral letters, clinical summaries and insurance narratives, the principles map onto the Ai workflow almost without modification. The problem is, most clinics haven’t done the translation yet, and their Caldicott Guardian is reviewing Ai use without a framework.
The eight principles and what they mean for Ai
Principle 1: justify the purpose. Every Ai use case should have a clear clinical purpose — draft a consultation note, summarise a specialist letter in plain English for the patient. Vague “use Ai wherever helpful” policies fail Principle 1.
Principle 2: don’t use patient-identifiable information unnecessary. This is where PII redaction in the Ai tool matters. NHS number, DOB, name, address should be stripped before the model sees clinical text. The Ai can still draft the note; it just doesn’t need the identifiers to do so.
Principle 3: use the minimum necessary. The Ai should see only the data relevant to the specific task — per-patient vaults so the Ai on one consultation can’t retrieve another patient’s record.
Principle 4: access should be strictly on a need-to-know basis. Per-user access controls, role-based scoping, and an audit log of who accessed what when.
Principle 5: everyone should be aware of their responsibilities. This is where the Ai policy comes in — clinicians need to know what they can and can’t do. We’ve published a free Ai policy template for UK SMEs that adapts for a clinical setting.
Principle 6: comply with the law. Specially UK GDPR, special category data handling, and Mental Capacity Act considerations where applicable.
Principle 7: the duty to share information can be as important as the duty to protect. This is actually relevant for Ai — patient summaries in plain English for the patient, referral letters to other clinicians, insurance narratives — all support good care, provided the sharing is controlled.
Principle 8: inform patients about how their data is used. If Ai is used in their care, they should be told — in a practical way, probably via the practice’s privacy notice.
The Caldicott Guardian Ai checklist
Basis principles above, six practical checks for your Caldicott Guardian. One, does the Ai tool use UK data residency? Two, is PII redacted before the model sees clinical text? Three, are patient records isolated per-patient, so Ai on one record can’t retrieve another? Four, is every Ai interaction logged with clinician ID, purpose, patient ID? Five, can the firm export or erase all Ai-assisted data per patient on request? Six, is the practice’s privacy notice updated to mention Ai use?
Apart from this, specially for GDC-regulated dental practices, check whether the Ai use supports the GDC Standards — principle 4 on maintaining and protecting patient information, principle 5 on working with colleagues, principle 6 on reporting concerns. Well-governed Ai tooling supports all three.
Where clinics get Ai wrong
The common failure mode is using general Ai tools for clinical work. ChatGPT, Claude, Gemini in their consumer form are UK-GDPR problematic for clinical text, regardless of how well the clinician writes the prompt. Specially for voice-note transcription with patient identifiers still audible, the risk is immediate.
The second failure is using Ai for clinical decision support without the evidence chain. Ai can flag, suggest, summarise — it cannot diagnose or prescribe. The clinician signs; the Ai supports. Apart from this, clinical decisions must remain the registered professional’s, with the Ai evidence as supporting material.
CQC inspections
Specially since 2024, CQC inspectors have started asking about Ai use during practice visits. The question is whether the practice has a framework for safe Ai use, not whether Ai is used at all. Framework means: written policy, governed tooling, audit chain, staff awareness, patient transparency. With those, Ai use becomes a positive indicator of practice maturity rather than a risk flag.
